Who Can Have Extracorporeal Shock Wave Treatment (ESWT) and How Is It Applied?

It can be applied to all patients with erectile dysfunction. The existing conditions of the patients do not constitute an obstacle to application. It has no known side effects. It is applied in 6 sessions in patients with mild to moderate erectile dysfunction and 12 sessions in patients with severe erectile dysfunction or diabetes. Sexual activity is not restricted after the procedure. The efficacy of the treatment begins to appear in 3 weeks following the last session, and this period may take up to 3 months.

How Does Extracorporeal Shock Wave Treatment (ESWT) Treat Erectile Dysfunction?

As a result of the microtrauma at the cellular level it causes in the tissues of the penis responsible for erection, it triggers the formation of new vessels by enabling release of various substances from these structures and activation of stem cells, activates tissue repair mechanisms and enables dissolution of microplaques. This leads to renewal of the nerves involved in erection in these structures. Penile blood flow improves thanks to newly formed vessels and renewed nerves, which clinically manifests itself as a strong, tight and sustainable, quality erection. The success rate has been reported to be 75%.

The procedure is painless. It has no known side effects.

Why Extracorporeal Shock Wave Treatment (ESWT)?

It solves the problem that causes erectile dysfunction. It is safe, it can be easily applied to every patient. It does not cause pain and there is no need for anesthesia for application. It does not require expensive prescriptions, medications to remember every time and surgery. It is an effective and safe treatment method for patients with erectile dysfunction who cannot use oral medication due to heart problems, diabetes or hypertension. It also allows the medication to be completely discontinued or its dose to be reduced in patients with erectile dysfunction who take medication for it. In this way, it also protects the patient from possible side effects of the medication and high medication dose. In addition, it is an effective treatment for patients with severe erectile dysfunction who do not respond to oral treatments (Viagra, Levitra, Cialis, etc.) and saves the patient from a serious surgery such as penile prosthesis placement.

Patients with mild to moderate erectile dysfunction and without any severe cardiovascular disease, diabetes or any damage to the nerves that enable erection constitute the patient group who will benefit the most from extracorporeal shock wave therapy. Concurrent oral medication (Viagra, Levitra, etc.) increases the efficacy of the treatment. Extracorporeal shock wave therapy (ESWT) is a proven effective and safe treatment method that can be applied to anyone at any age who wants better erection and ideal sexual performance.

Penises

Penises come in many different shapes and sizes and although the average penis size is anywhere from 5.1-5.5 inches, upto 45% of men report that they are unhappy with the size of their penis and wish they had a larger or thicker one. Those who are worried about the size of their penis often complain about having low-self exteem and performance anxiety which has a direct influence on how they perform in the bedroom. At Amerikan Aesthetiic, we have two main procedures which may help alliviate some of that anxiety you may have.

Post-op care

• • You should expect some pain and soreness after the surgery but we will provide you with the necessary painkillers to deal with this.
• • The surgical dressings that your penis are covered in will need changing on a regular basis as suggested by your surgeon.
• • Once you have arrived back home try to rest as much as possible and try not to over exert yourself.
• • You should massage your penis gently with vaseline each day to prevent the build up of nodules on the shaft and to prevent the penis from becoming lop-sided.
• • Keep the area clean while healing but be careful not to get water onto the area. Instead of having a shower you should have a sponge bath.
• • Do not go swimming or participate in any sports for at least a month.
• • Avoid heavy lifting or any other strenuous activity for around a month post-op.
• • You should not have sexual relations for the first 6 weeks after the operation (sorry gentlemen that also includes masturbation)
• • It can take up to 3-6 months before any results can be seen.

Penis Fat Transfer (Penile Girth)

Penis fat transfer (also known as fat transfer phalloplasty) is where the surgeon removes fat from one area of the body (mainly the tummy area) and then transfers it to the shaft of the penis to provide more girth (generally by up to 30-40%). By using the patients own fat we can remove the risk of an adverse allergic reaction that is often associated with synthetic fillers that are also available on the market.

Post-op care

• • For the first 2 weeks after the procedure you should expect bruising, swelling and tenderness.
• • Avoid long walks, standing for a extended periods and other strenuous activities for about a week after the procedure.
• • 2 weeks after the procedure you may resume light activities and sexual intercourse (some patients complain of pain while they are erect for upto a month after the procedure but this is normal.
• • Final results should be evident after 5-6 months but some patients may need multiple sessions to achieve their desired result.

Penuma® Silicone Implants (for girth)

The penuma® silicone implant may increase your penise’s girth by 1-2inches (2.5-5cm). There have also been reports of increased length in both flaccid and erect penile length, but this is totally unpredictable and is down to each individual’s biological makeup.The implant is made of a medical grade silicone similar to that found in other procedures such as chin implants and does not contain any fluids or gels. The implant is inserted through an incision made in the scrotum when once healed, should bareley be visible due to the texture of the skin around the scrotum. Once the implant has been positioned, correctly it will be stitched into place just behind the head of the penis, so that it too can stretch when the penis becomes erect. A drain may have to be worn for 2-3 days after the operation so that any excess fluids resulting from the operation may be removed. Penuma® implants may be removed surgically too, anytime the patient feels he no longers wants one.

• • The drain will be in place for approximately 2-3 days or until all the necessary fluids have been drained. Our nurse will direct you how to empty the drain of excess fluid when need be.
• • For the first 6 weeks avoid any activities that may put stress on the penis such as continuously moving it, touching it, or sexual activities such as masturbation and intercourse.
• • Place an ice pack packed in a cloth on the scrotum for a 20 minute period once every half hour throughout the waking day.
• • You will experience pain and discomfort for around 2 weeks after the procedure and söre for up to 6-8 weeks.
• • Swelling may last up to 6-8 weeks and is a normal part of the healing process.
• • Shower once a day and wash the penis, scrotum and incision with natural soap and be careful to pat dry.
• • Nighttime erections will be painful and uncomfortable until the recovery process is complete.

Penile Implants (for erectile disfunction disorders)

A penis implant is a device that is inserted inside the penis to help men with penile dysfunction achieve an erection. There are 3 types of penile implants, semirigid, inflatable, and both types require surgery. It is important to note that while a penile implant will help you achieve an erection, it will not necessarily increase sexual desire, libido or sensation. In addition to this, you should remember that a penile implant will not increase the size of your penis either.

Types of penile implant

1) Inflatable Implants

These are the most common types of penile implant and can be inflated to cause an erection to occur and then deflated after use. There are 2 types of inflatable implant:

Three-part implants: These use a saline solution which is concealed in a resevoir under the abdominal wall and a pump and release valve is placed inside the scrotum and also two inflatable ballons are concealed in the shaft of the penis. In order to make your penis erect, you simply pump the fluid from the resevoir into the inflatable ballons and when finished you press the release valve to drain the saline solution back into the reservoir. This type of implant provides the most realistic erection and total flaccidity when deflated but as it also has more parts that could malfunction.

Two-part implants: These work in a very similar way to three-part implants, but instead of having the fluid reservoir implanted separately in the abdomen, it is part of the pump that is implanted in the scrotum. This type of implant only provides partial flaccidity when deflated and provides a less firm erection than its three-part counterpart.

2) Semirigid Rods:

These devices are always firm. When needed the penis can be bent up and away from the body for sexual intercourse or down and toward the body for concealment. A positionable penile implant is such a device and consists of a number of segments held together with a spring on each end and is able to hold its position better than any other type of semirigid device. This type of device has lesser chance of malfunction due to its limited number of parts and its easy to us efor those who have limited manual dexterity. On the downside, it usually results in a penis that is always slightly rigid and can result in a penis that is difficult to conceal under clothing.

Post-op Care:

• • For the first 2-3 days after the operation, get as much bed rest as you can
• • The dressing that is covering your penis should be kept on for 2-3 days and you will instructed how to gently remove the dressing when the time is right.
• • Your scrotum should be supported with the penis elevated in an upright position. The use of a jock strap or tight underwear is enough to provide gentle compression to the scrotal area.
• • Once the dressing is removed you should shower daily being sure to apply soap gently by hand in order to clean the area and then patted dry.
• • Do not immerse the wound in water (bath,swimming pool) until the wound has completely healed.
• • Refrain from any strenuous activities for the first 2 weeks.
• • For the first 6 weeks avoid any activities that may put stress on the penis such as continuously moving it or sexual activities.
• • Use a mirror to check the wound area. There may be some post-op swelling and some of the skin in the area may be black or blue; this is totally normal and nothing to be concerned about. However, if you have a fever, and increased swelling and tenderness with a creamy substance seeping through the sutures, you may have an infection and you should get in contact with your patient coordinator for further advice.

In 2020, a study found that the average penis length is between 5.1 inches and 5.5 inches (however, once volunteer bias was accounted for the researchers concluded this number could be lower.
The average girth for a flaccid penis is 3.66 inches and while erect 4.6 inches.
Around 85% of people over estimate what the average penis size is.
95% of penises fall into the average range.
5% of people have a penis length of 3.94 inches when erect.
In a popular womens magazine 77% of people stated that penis size does matter. In the same study 64% of women stated that girth was more important than length.

Liposuction is a procedure that can help eliminate those pockets of fat and thus help reshape your body. During this procedure a device known as a cannula is inserted in the fatty area below the epidermis so that it can suck out any unwanted fat. The areas treated most common by our male patients include the tummy, waist, back, chest chin and arms.

Six pack fat transfer

Are you tired of the incredible amount of exercise that is required to maintain a well defined six pack? Then a six pack fat transfer may just be the thing for you. To begin with, your surgeon will remove as much fat that is needed from either your tummy, waist or thighs using liposuction and then reinject this fat directly into the muscles of the abdomen to give the appearance of a natural looking six pack without the exercise and sweat.

Tummy tuck, thigh lift and arm lift

Occassionally, a patient may have so much fatty build up in a particular area that a simple liposuction procedure is no longer enough as by doing liposuction alone, the patient will be left with an excess of saggy skin. A tummy tuck, thigh lift and arm lift are all procedures that remove this excess skin after liposuction has been applied so that the patient is left with skin which is much tighter and less saggy.

Hair transplant

Whether you are just thinning on top or have gone totally bald a hair transplant is a procedure which healthy hair follicles (usually from the back of your head) are extracted and then replanted back into the area that requires follicle regrowth. At American Aesthetic, we have 2 types of porcedure available; the first of which is the Sapphire FUE method, in which channels are opened using a blade made from sapphire and then we have the DHI method which uses a special implement to reinsert the healthy follicles into the scalp called a “choi pen”. Hairtransplants can also be used to address issues with facial hair such as the beard too.

Blepharoplasty

As we age, the skin around the eyes begins to sag and lose shape often making our eyes look puffy and tired. Blepharoplasty is a procedure in which this lose skin is removed either on the upper eyelid, lower eye lid or both reswulting in eyes which look fresher and more youthful.
